Landmark Connect builds upon Landmark’s home-based medical program. It is a fee-for-service (FFS) model designed to provide primary care services focused on proactively managing residents’ health conditions and ensuring personalized care coordination while they reside in facilities such as:
- Assisted Living Facilities (ALF)
- Continuing Care Retirement Communities (CCRC)
- Independent Living Facilities (ILF)
- Skilled Nursing Facilities (SNF)
- Other Long-term Care (LTC) facilities

LANDMARK CONNECT OVERVIEW
Landmark Connect providers assist with the heavier clinical load associated with frailer patients. Our customized quality care plans are designed to deliver better care for residents and improved STAR quality metrics for facilities. Key areas of focus:
WELL VISITS: Our providers can assume the role of primary care or attending clinician, rounding in your facility to care for and monitor residents. For families of residents, this offers the opportunity to connect frequently with the provider and discuss questions in person.
EDUCATION: Our providers educate staff, family and other involved providers on resident conditions, and address palliative care discussions when needed. Being involved and knowing how best to support a loved one helps give residents’ family members peace of mind.
URGENT CARE: Our providers are embedded in your facility and are onsite when urgent needs arise. Because they know both patient history and wishes, they can quickly assess needs and determine appropriate care paths. This may reduce hospital re-admissions or transfers back to acute settings.
A SHIFT TO VALUE OF CARE
Landmark Connect shifts from volume of services to value of care by creating new opportunities to address gaps in management of a frail patient population. By embedding a Landmark care team in a living facility, we can build relationships with the residents, families and facility staff to improve early identification of changes in residents’ status and better monitor ongoing treatments.
By knowing the residents’ history and wishes, Landmark’s providers can quickly assess needs and determine appropriate care paths. We can keep everyone informed of any medical needs or changes in conditions, and educate the staff and families to deliver more effective care and communication.
This highly collaborative work:
- Improves the quality of medical care and outcomes for residents,
- Helps to reduce avoidable trips to the hospital, and
- Improves care transitions across providers for more coordinated care.
THIS HIGH-TOUCH CARE IS AVAILABLE AT NO ADDITIONAL COST TO RESIDENTS, OUTSIDE OF THEIR STANDARD INSURANCE CO-PAYS AND DEDUCTIBLES.
