Patients qualify for Landmark’s in-home medical services through their health plan or provider group. Eligible patients have complex health needs, including multiple chronic conditions, and live in our service areas.

If so, then you are eligible.

If not, the best way to find out if you are eligible for the Landmark program is to call Landmark at the phone number listed for your health plan or provider group on our Locations page. Be sure to have your health plan ID number available to help verify eligibility.

1. You will receive a letter from your health plan or provider group introducing Landmark in the mail.
2. A Landmark team member will call you to discuss our services and schedule time for a Landmark provider to visit you at home.
3. During your in-home visit, a Landmark doctor, nurse practitioner, or physician assistant will ask about your health goals, discuss Landmark’s services and answer your questions. They will go over the forms that allow Landmark to treat you, similar to those you sign in your doctor’s office. Landmark will review your medical history and schedule a future visit.
4. Once you are a Landmark patient, you can reach a Landmark provider any time of the day or night, including weekends and holidays.
5. Notes from your Landmark visits are sent to your primary care doctor. Landmark is an added layer of support at home, and does not replace the care you receive from your other doctors.

Yes. Landmark does not change your current health plan or benefits in any way. You keep your primary care provider and can still access the physicians and providers within your network.

Yes! If you are not eligible now, it doesn’t mean you won’t become eligible for Landmark in the future. Feel free to periodically check-in to see if you’ve been added to the Landmark patient roster. You can also call your health plan to see if you qualify for another in-home medical care program they may offer.
