Blue Cross NC, Landmark to Deliver Home-Based Medical Care to Customers with Complex Chronic Conditions

Huntington Beach, CA – April 6, 2018 – Blue Cross and Blue Shield of North Carolina (Blue Cross NC) today announced a new business partnership with Landmark to deliver home-based medical care to Medicare Advantage (MA) customers with complex chronic conditions, such as diabetes, COPD and congestive heart failure, beginning June 2018. Through this model of care, Blue Cross NC and Landmark will bring in-home medical care to approximately 6,500 customers having complex chronic conditions and located in the Triangle, Triad, Charlotte and Fayetteville regions of the state.

 

Our members would rather be at home instead of in the emergency room,” said Dr. Patrick Conway, President and CEO of Blue Cross NC. “Through this partnership, we are now able to offer an added level of care to help members manage their conditions at home, and stay ahead of a crisis that would otherwise put them in the hospital.” 

 

The program will be available for Blue Cross NC customers with multiple chronic conditions who are currently enrolled in a Blue Cross NC Medicare Advantage plan. The program is available at no additional cost for eligible members. Landmark care teams will deliver home-based medical, behavioral and palliative care and social support services to eligible Blue Cross NC customers. This supports the Blue Cross NC vision to improve health outcomes, lower costs, and deliver exceptional service to all North Carolinians.

Blue Cross NC is the only insurer in North Carolina to offer this model of care. The Landmark program complements clinic-based primary care and works to support the primary physician’s care plan with services provided in the patient’s home. The targeted launch date of the program is June 1, 2018. Eligible Blue Cross NC customers will be contacted by Landmark to discuss their health care options and needs.

 

Landmark will offer Blue Cross NC customers 24/7 telephone support where they can reach a Landmark provider after-hours, on weekends and holidays, including times when their regular doctor’s office is closed. When needed, a provider can be in a customers’ home within the hour.
