As your parents age, you can’t always be there for them. Sometimes hundreds, if not thousands of miles separate you from an elderly loved one. That’s when Landmark can help. Native New Yorker Tobi Steinberg found that out when she signed up her aunt Anne for Landmark care.
Tobi now lives in Colorado Springs, but Anne, the aunt who raised her, still lives in the Bronx. At 92, Tobi knows Anne needs more help with her health than she can provide from 1,800 miles away. When Tobi heard about how Landmark delivers care in the comfort of your home, she signed her aunt up. Skeptical, she watched her aunt’s first Landmark visit through cameras set up in the Bronx apartment. To find out what happened, watch the video.
“I’ve been a nurse practitioner working in primary care for over 30 years, mainly with the elderly. I originally went to graduate school with the idea of working in the home as I’ve always loved home care and its unique perspective. Although I absolutely loved my previous practice, colleagues and patients, I was only able to make infrequent although regular house calls. This reinforced my belief that house calls to fragile elderly are so much more valuable than an office practice and result in better outcomes.”
What Does Landmark Cost?
For many patients, Landmark’s services are available at no cost. We are able to do this because of our risk-based partnership model with health plans that moves us beyond the traditional fee for service system. We work with health plans’ complex patient populations, offering an additional layer of care and support to help keep patients healthy and home, avoiding unnecessary emergency room or urgent care visits.
What Care Does Landmark Provide?
Landmark can treat these conditions at your home:

- COPD and Asthma
- Urinary tract infections (UTIs)
- Cuts, wounds and rashes
- Cellulitis and abscesses
- Nausea, vomiting, and gastrointestinal issues
- High blood pressure
- Fever, colds, pneumonia, upper

- Administration of IV fluids
- In-home lab, X-rays and ultrasounds
- IV and injectable medications, including antibiotics for serious infections
- Prescriptions and medication review
- In-home pharmacist, dietitian and social worker
- Referrals for home oxygen, durable medical equipment, home health services and hospice
- Palliative care consultations
- Catheter insertion and removal
Landmark’s community-based providers and multidisciplinary care teams live and work near our patients. We go to them – where they reside and when they need care. We connect as a team in our local offices to collaborate on patient cases in order to see and treat the whole patient.
