Landmark Health and Ceresti Health Announce an Innovative Program Focused on Enhancing Care for Patients with Dementia

Program empowers caregivers of high-need Medicare Advantage patients with coaching, education and support

HUNTINGTON BEACH, Calif., and CARLSBAD, Calif., November 16, 2017 — Landmark Health and its subsidiaries (“Landmark”), specializing in home-based medical care, and Ceresti Health, a company utilizing digital solutions to enhance personalized patient engagement, today announced the successful completion of patient enrollment in a controlled study of the Ceresti PREVENT program in a high-need Medicare Advantage population. Clinical and other outcomes from qualified Landmark patients enrolled in the Ceresti care coaching, education and support program will be evaluated against a control population.

Ceresti has created an evidence-based, 12-week program delivered on a dedicated computer tablet and supported by remote caregiver coaches who provide guidance and support throughout the entire program. The Ceresti PREVENT program leverages behavioral science and deep personalization tools to empower informal family caregivers and provide the best possible care for high-need patients with dementia. Through daily engagement and personalized interaction, caregivers develop the knowledge, skills and confidence required to address the specific challenges and needs associated with Alzheimer’s and dementia patients, which can often include management of multiple chronic conditions.
