Every May, we celebrate Mental Health Awareness Month.
Optum Care Network Washington has teamed up with Landmark to deliver in-home medical care to members with multiple chronic conditions at no additional cost.
Health Care that Comes to You
Stay home when you don’t feel well. Medical care can now come to you in the comfort of your own home. The Landmark team doesn’t replace your primary care doctor or other specialists. You continue to see the providers you know and trust, while getting additional support from Landmark’s in-home medical team of doctors, advanced practice providers, pharmacists, social workers and others.
Landmark Services
Landmark providers bring medical care to you in person, over the phone or through video. Landmark is available at any time of the day or night, including when your regular doctor’s office is closed.

Landmark can also assess your home for fall hazards and other safety considerations. We help you stay independent at home for as long as possible.
Eligibility
If you are eligible for Landmark services through Optum Care Network Washington, you will receive a letter enclosed with a brochure explaining the services available to you.
