Healthcare that Comes to You

Stay home when you don’t feel well. Medical care can now come to you in the comfort of your own home. The Landmark team doesn’t replace your primary care doctor or other specialists. You continue to see the providers you know and trust, while getting additional support from Landmark’s in-home medical team of doctors, advanced practice providers, pharmacists, social workers and others.

Landmark Services

Landmark providers bring medical care to you in person, over the phone or through video. Landmark is available at any time of the day or night, including when your regular doctor’s office is closed. Visits are based on your needs and your schedule, and can be used for:
• In-home medical, mental health, and social care
• Urgent care visits
• Routine visits
• Hospital post-discharge visits
• 24/7 telephone support

Landmark providers can address urgent needs, such as:
• Cuts, wounds, and rashes
• Fevers, colds, pneumonia, upper respiratory infections
• Urinary tract infections
• IV fluids, IV medications, catheter insertion/removal

Landmark can also assess your home for fall hazards and other safety considerations. We help you stay independent at home for as long as possible.

Eligibility

You may be eligible for Landmark’s services if you have a combination of any of the following conditions:

• Atrial fibrillation
• Cancer
• Cerebrovascular disease
• Chronic kidney disease
• Coronary heart disease
• Diabetes
• Disabling conditions
• End-stage renal disease
• Fluid and electrolyte disorders
• Heart failure
• Pressure ulcers with necrosis: stage 4
• Protein-calorie malnutrition
• Pulmonary disease
• Severe chronic liver disease
• Vascular disease
