Landmark Story: Patient & Provider
Caring for patients in the greater Albany Area
I’ve been a nurse practitioner working in primary care for over 30 years, mainly with the elderly. I originally went to graduate school with the idea of working in the home as I’ve always loved home care and its unique perspective. Although I absolutely loved my previous practice, colleagues and patients, I was only able to make infrequent although regular house calls. This reinforced my belief that house calls to fragile elderly are so much more valuable than an office practice and result in better outcomes.
I had become aware of Landmark as several of our office patients also had Landmark providers. As I investigated the model and the company, it became clear to me that my beliefs and values about caring for the elderly were echoed in Landmark’s vision and mission of doing what’s right, even when it’s not always easy.
As a nurse practitioner in the home, I can be mindful, intentional and proactive making more of a difference in the lives of the frail elderly in my own community, thereby transforming healthcare, one person at a time.
The Landmark model calls to mind Margaret Mead’s words, words to live by: “Never doubt that a small group of thoughtful committed citizens can change the world. Indeed, it is the only thing that ever has.”
