Huntington Beach, CA, February 13, 2020 – Landmark Health and its affiliated medical group (Landmark) and Blue Cross & Blue Shield of Rhode Island (BCBSRI) have come together to enhance access to care and coordination for BCBSRI Medicare Advantage members managing multiple chronic conditions.
Landmark’s physician-led medical group gives patients access to a team of specialists who provide highly coordinated and timely care. This team approach helps:
- treat routine and urgent medical needs in the home,
- identify and address social determinants of health,
- assist patients and caregivers with advanced planning documentation,
- provide education on proactive condition management, and much more.
The cumulative effect of this high-touch care is reduced unnecessary hospitalizations and fewer visits to the emergency room, along with lower mortality rates and costs.
“Landmark providers—Complexivists®— help identify problems early to proactively manage conditions through education as well as treatment,” states Dr. Michael Le, Chief Medical Officer and co-founder at Landmark. “Keeping patients stable in the comfort of their home is our goal.”
Landmark Health will support BCBSRI’s HouseCall by Blue program, which is a voluntary program offered at no cost to eligible BCBSRI Medicare Advantage members. It is supplemental care for people with complex medical and behavioral needs, working in tandem with patients’ established care teams, caregivers and community services.
Landmark Health and its affiliated medical groups (Landmark) partner with health plans and delivery systems to bring patient-centric, in-home care to complex and chronically ill populations. The company bears risk for more than 100,000 lives across 14 states. Landmark provides access to care 24/7 to patients and their families. Its value-based model relies on fully-employed, local multidisciplinary care teams to help drive long-term outcomes for patients by bringing medical, behavioral, social and palliative care to individuals, where they reside and when they need it. Landmark is bringing back the house call. Learn more at www.landmarkhealth.org.
