July 2, 2019 – Landmark Health and its affiliated medical groups (Landmark) launched a new website. The site features improved speed, usability and rich content specific to the audiences Landmark serves.
As a risk-based provider group, Landmark partners with clients – including health plans, care delivery systems, and groups who bear risk over patient populations – to bring high-touch, longitudinal care to patients with complex, chronic conditions. We’re bringing back the house call.
The new site offers clear paths for:
- Landmark-eligible members, patients, prospective patients, and their families
- Potential clients
- Primary care providers
- Potential recruits
Changing the Status Quo
The enhancements to Landmark’s website are intended to help all people who interact with Landmark better understand who we are, what we do, and educate audiences of our values to better care for our country’s most vulnerable patients.
As the population ages, a new delivery of care is required to meet the needs of those managing chronic conditions, behavioral health conditions, social isolation, reduced mobility, and beyond. For our patients, getting to their doctors’ appointments can be a challenge, resulting in overuse of ambulatory services such as the emergency room (ER). Bringing care to these patients where they reside routinely and urgently can help them stay well at home, reducing avoidable ER visits and hospital admissions. Not only does this save costs, but it also reduces patient stress and potential longer-term care needs like rehab or nursing facility stays.
It takes a community to care for the most vulnerable. Landmark works in collaboration with health plans, primary care providers – our patients keep their regular doctors – and community-based services. Our care is supplemental and usually at no cost or co-pay to patients.
