Landmark’s local mobile doctors (along with other providers), are serving the needs of chronically ill people in our communities. Not only does high-touch, in-home care improve access to care for patients, it also improves communication among all those involved in a patient’s care – such as family members or other caregivers.
On a typical day, Dr. Dove may see up to 10 Medicare patients where they reside. Compared to facility-based care doctors who see 20 to 25 patients every day, this reduction in patient volume allows for more quality time spent with each patient to fully understand their conditions and life goals.
“Some patients, their goal is plain and simple, they would like to stay at home,” Dove said. “They don’t want to go to the hospital, they don’t want to go to the doctor’s office.”
Landmark’s providers have the time to get to know their patients, their home life and their other caregivers and providers. This in-depth view helps to focus on long-term patient care and outcomes, with special attention put to staying on top of conditions in order to keep the patient as healthy as possible at home.
