I’m honored to lead the Landmark team through our next chapter of growth. – Chris
Chris will succeed Nick Loporcaro, who has led Landmark for the past three years, seeing the organization through significant growth extending into 18 states with more than 175,000 covered lives.
“The future of home-based medical care has never been brighter,” states Nick Loporcaro. “The healthcare industry and consumer marketplace are seeing the value of medical care delivered into the home, especially for our frailest patients. Chris has been integral in building Landmark and developing our growth and partnership strategies. I’m confident he’s the right person to lead Landmark through this exciting new phase.”
Chris joined Landmark in 2017 as the vice president and general manager responsible for launching the New England market. Most recently, Chris has served as Landmark’s head of corporate development, responsible for our growth strategy, strategic partnerships, mergers and acquisitions, and public policy. Prior to Landmark, Chris was a principal at Innosight, a healthcare focused growth strategy consultancy and co-founder of Predilytics, a healthcare analytics business.
“Landmark’s value-based clinical model is actively transforming health care in America,” states Chris Johnson. “By extending primary and urgent care into the homes of patients with challenging illness, we’re helping seniors age in place, surrounded by family and friends. It’s life-changing and inspiring work, and I’m honored to lead the Landmark team through our next chapter of growth as we look to double the number of patients we serve and expand our footprint to 22 states in 2022.”
Landmark is revolutionizing care delivery for seniors with chronic illnesses, bringing home comprehensive medical care that considers patients’ physical, mental, and social needs to help reduce emergency room visits and give patients more healthy days at home. Landmark doctors and advanced practice providers work in coordination with patients’ primary care physicians and specialists.
Since 2014, Landmark Health and its affiliated medical groups (Landmark) have delivered comprehensive in-home medical care to older adults. Specialized in complex chronic care, Landmark’s physician-led multidisciplinary teams work with patients’ existing healthcare networks to bring medical, behavioral, and palliative care, along with social services, to individuals where they reside. As one of the nation’s largest value-based healthcare companies, more than 175,000 people across 18 states and 52 communities can access Landmark’s care. Most critically, Landmark is bending the cost curve in health care by reducing avoidable ER visits and hospital admissions, while extending the lives of patients.
