This national award was created to honor Dr. Martin L. Block, who dedicated his life to improving the quality of healthcare through clinical innovation and excellence. “Under Dr. Broderick’s direction, a team of interdisciplinary clinicians, led by physicians, nurse practitioners and physician assistants, provides care to patients where ever they reside, whenever they need it. This transformational model of health care focuses on identifying the drivers and triggers that often result in avoidable hospital admissions, avoidable emergency room visits, and medical non-compliance that often increase the risk of frail patients. Since the implementation of these services in Albany, the Landmark Health team has made over 14,712 visits to patient homes, including 2,816 urgent visits.
Dr. Broderick has built on Landmark Health’s transformational clinical model by continuously challenging his team of medical providers to drive meaningful patient outcomes… Broderick piloted and instituted a new medical provider role unique to Landmark Health, called the Urgentivist; this role is a highly skilled medical practitioner that makes urgent house calls any day of the week, at any hour. An Urgentivist is equipped with the skills, knowledge and medical tools to make in-home diagnoses, begin a regimen of treatment, and stabilize a patient that might otherwise require hospitalization.
As a leader, Dr. Broderick has been instrumental in re-engineering the mindset of his team. Many of the providers that joined Landmark Health came from a fee-for-service delivery system, where volume is rewarded.
