An Overview of Home-Based Primary Care: Learning From the Field
The future of home-based primary care is strong. The need will continue to grow as the population ages and this model of care delivery is proving to improve quality of care, increase value to patients and decrease costs.
Landmark was invited to participate in an industry case study on home-based primary care conducted by The Commonwealth Fund. The study covers various business models designed to deliver value-based care and how cost-savings are achieved. But more importantly, it has key insights that resonate with our experiences in caring for those who are homebound or home-limited, including:
- the importance of interdisciplinary teams,
- the benefits of urgent care visits,
- the value of behavioral health and palliative care specialists, and
- providing communication and support to family members and caregivers.
The future of home-based primary care is strong. The need will continue to grow as the population ages and this model of care delivery is proving to improve quality of care, increase value to patients and decrease costs.
