Becoming a Landmark Patient

Landmark is available to patients who qualify for the program through their health insurance company. If you, or a loved one, are a member of one of these plans, call to see if you qualify for Landmark.

What People Are Saying About Landmark

“I was 55 when I had a stroke. It left me with limited mobility. I couldn’t do work. I could not take care of myself as well. I felt so isolated, angry and confused. My Landmark provider, Mary, helped me get physical therapy and occupational therapy here in my home, because then it’s easy for me. I don’t have to find a ride to go to get care. It’s amazing, because the treatment is done here at home.”

Landmark Patient, Milbrae, CA

“Our goal is to keep people healthy at home. Sometimes when you go to the emergency room, there is this snowball effect of all of a sudden you’re staying overnight in the hospital, then you’re staying a week, next thing you’re in a nursing home. We work to prevent it right at the beginning, that’s the hope.”

Landmark Advanced Practice Provider, Bay Area, CA

“Landmark helped me see another birthday. Last month I had an asthma attack. It was by far one of the worst things that has happened to me. Instead of heading to the emergency room, I called my Landmark providers. They prescribed medication to lessen the symptoms of the immediate attack as well as tweak other medications to reduce a chance of a reoccurrence. Now I can get back to doing what I love – impersonating a professional wrestler at community events. Landmark keeps me healthy enough to continue giving back.”

More About the Landmark Program

“Landmark Health has been a life saver for me. Before Landmark, I had as many as 9 doctors and specialist providing care. However, none of these providers seemed to be communicating with each other. I also had difficulty scheduling multiple appointments with my doctors. I was offered the Landmark program and immediately jumped at the chance to be seen in my home rather than going to the doctor’s office.
When I first started seeing my provider the relief was so immense because finally I had somebody else looking at my medical care. It was such a load off my shoulders. She heard me, she listened to me. They look at everything with regards to my care, not just the immediate issue. I now have a team of people coordinating my care and they come to me, in my home. I am so grateful to be a Landmark patient.”

Julie
Landmark Patient

“I’ve been a nurse practitioner working in primary care for over 30 years, mainly with the elderly. I originally went to graduate school with the idea of working in the home as I’ve always loved home care and its unique perspective. Although I absolutely loved my previous practice, colleagues and patients, I was only able to make infrequent although regular house calls. This reinforced my belief that house calls to fragile elderly are so much more valuable than an office practice and result in better outcomes.”
